Download the form below, print it, and fill it out to the best of your ability. You can also pickup a form at Man O’ War Harley-Davidson from our H.O.G Manager. You can pay your dues at the store when you complete the form.

As a Man O’ War H.O.G Member you are an extension of our personal brand and our message of riding Kentucky! You will represent Man O’ War Harley-Davidson as an advocate and a member of our family everywhere you go.
Our H.O.G Chapter is very involved in everything happening at the shop, from events to rides, and helping us promote the riding community in Kentucky and beyond.
